A RARE CASE OF GIANT JUVENILE FIBROADENOMA OF THE BREAST IN A 19 YEAR OLD NIGERIAN GIRL: A CASE REPORT

Abstract

Background: Giant juvenile fibroadenomas are rare yet significant benign breast tumors predominantly affecting adolescent and young women. They are extremely rare, making up only 0.5-2% of all fibroadenomas. Due to their rarity, there are no set guidelines for their management and there are several different approaches to management

Case presentation: This case report outlines the clinical presentation and management of a massive juvenile fibroadenoma in a 19-year-old female, contributing valuable insights to the limited literature on this uncommon entity. The patient presented with a progressively enlarging right breast mass, prompting clinical suspicion and subsequent surgical management. Under general anesthesia, the surgical excision revealed a 22 cm x 16 cm x 8 cm mass, weighing 2820 grams, approaching the upper limits of previously reported masses. The surgical technique emphasized preservation of the nipple-areolar complex to optimize cosmetic outcomes and preserve breastfeeding function. Histologically, the tumor exhibited biphasic stromal and epithelial features, consistent with juvenile fibroadenoma. Surgical intervention remains the cornerstone of treatment, aiming to alleviate symptoms, ensure cosmesis, and rule out malignancy. 6-month follow-up showed a satisfactory outcome with minimal impact on breast development and favorable cosmetic results.

Conclusion: This case underscores the importance of early detection, accurate diagnosis, and appropriate surgical management in achieving optimal clinical outcomes while addressing patient concerns related to body image and long-term breast health. It also adds to the growing body of literature on giant juvenile fibroadenomas, providing valuable insights into their clinical presentation and management strategies.
