PHYSICAL DISABILITY INDEX AND DEPRESSION AMONG STROKE SURVIVORS IN PORT HARCOURT, NIGERIA

Abstract

Background: Depression in stroke survivors has been associated with dire consequences including decline in productivity, joblessness, and even suicide or suicide attempts. This study sought to investigate the relationship between depression and physical disability among stroke survivors in Nigeria in a bid to promote holistic patient care.

Methodology: A hospital-based cross-sectional study involving 110 adult out-patients with stroke at the neurology clinic of the University of Port Harcourt Teaching Hospital (UPTH) was employed in this study. Physical disability was determined using the modified Ranking Scale (mRS). Depression was diagnosed based on the Structured Clinical Interview for Diagnostic and Statistical Manual of Mental Disorders (DSM-SCID). Inferential statistics employed Chi square and Mann-Whitney U statistics at 0.05 alpha level.

Results: The stroke survivors had a mean age of 54.49±9.33years. Seventy-one (64.5%) were males. The mean mRS (physical disability index) score was 2.81±0.55, and the median mRS score was 3. A total of 39 stroke survivors had depression (35.5%). The depression rates were 10.3%, 43.8% and 50.0% for survivors with mRS of 2, 3 and 4 respectively (Chi Square for trend=9.362; p-value=0.02). The mean rank of MRS score was significantly higher among depressed in comparison to the nondepressed (Mann-Witney U=988.50; p-value=0.003).

Conclusion: Depression and physical disability are linked among stroke survivors in Nigeria. The assessment of depression among stroke survivors at neurology out-patient clinics is advocated especially in those with moderate to severe forms of physical disability in order to improve clinical outcome.
