PRIMARY SEGMENTAL VOLVULUS OF THE SMALL INTESTINE IN A 3 YEAR OLD MALE: A CASE REPORT AND REVIEW OF THE LITERATURE

Abstract

Background: Primary volvulus of the small intestine is a rare cause of intestinal obstruction. This results in delayed diagnosis and a significant increase in morbidity and mortality. The paper takes a panoramic look at the aetiopathogenesis, diagnosis and treatment options available for this disease entity.

Case presentation: A 3-year-old male child presented with a 3-day history of severe abdominal pain and vomiting. Ultrasonography was not diagnostic and surgery revealed 45 centimetres of twisted gangrenous small bowel without any obvious aetiology. The twisted, gangrenous segment was resected with anastomosis of the remaining viable small bowel. He was discharged after a stormy 25-day stay in hospital.

Conclusion: It is recommended that primary volvulus of the intestine be considered in differential diagnosis of intestinal obstruction in children because early detection can be lifesaving. In addition, the importance of early presentation in hospital cannot be overemphasized and patient education is invaluable in this respect.
